Hospital Discharge 

When an individual is discharged from hospital, a well-coordinated transition is crucial to ensure a safe and smooth return home. We can be that person to support you and take the lead with assessing and planning the discharge from hospital.  

 

We can support you with the family meetings, liaising with the medical team, the ward staff and the discharge planners and take the worry, pressure and stress away, we can provide Comprehensive Care Planning: Prior to discharge, a tailored plan is developed in collaboration with health professionals. This plan addresses medication management, follow-up appointments, and any necessary rehabilitation or home care services.

Coordination with Community Services: Linking the individual to community health services and local care providers helps ensure continued support. This network may include general practitioners, home care aides, and specialised support services relevant to the person’s recovery needs.

Clear Communication: Open lines of communication between hospital staff, community providers, and the individual (along with their carers) are essential. This transparency ensures that everyone involved understands the care plan and can act together to address any potential issues.

Empowerment and Independence: The ultimate goal is to empower the discharged individual to regain independence while ensuring their well-being. Regular reviews and timely adjustments to the care plan help safeguard against setbacks.

This integrated approach not only promotes a seamless transition but also contributes significantly to long-term recovery and improved quality of life.

Terms of Service:

All fees will be payable in advance of scheduling an appointment.

Work outside the scope of this agreed package will be charged at an hourly rate of £90.

Travel is charged at the HMRC standard rate of 45p per mile.

Travel time is not charged for any journey that's pre agreed; thereafter, it is charged at £55 per hour.

All other travel expenses, such as parking and train fares, will be billed at cost.

Payment Terms.

Where we identify that an individual being assessed may have significant difficulty understanding the terms of our service agreement, payment for our services must be made by the referrer or a designated third party, using their own funds.

We reserve the right to decline payment from any individual we believe may lack the mental capacity to comprehend our terms and conditions. Furthermore, we do not endorse third parties accessing the funds of individuals who may not have the mental capacity to manage their financial affairs, unless they possess the requisite legal authority to do so, in accordance with Section 42 of The Care Act 2014.

Services

Option 1: Rapid Response Discharge Package - £900

What it Includes:

  • An initial hospital visit and assessment within 48 hours of instruction.

  • Liaison with the hospital's multi-disciplinary team to gather all relevant information.

  • A comprehensive Care Act assessment and a detailed support plan tailored for a safe discharge.

  • Rapid-response brokerage support to identify and help secure a suitable package of care at home or a placement in a residential/nursing home.

  • Support and guidance for the family throughout the entire process.

Option 2: Complex Discharge & Advocacy Package - £1300

    • Disputes with the hospital team over the proposed discharge plan.

    • Potential eligibility for NHS Continuing Healthcare (CHC) that needs to be explored or challenged.

    • Complex family dynamics or safeguarding concerns.

    • Questions around the person's mental capacity to make decisions.

    • Sourcing a highly specialised service for conditions like advanced dementia or complex physical disabilities.

  • What it Includes: Everything in the Rapid Response package, PLUS extended advocacy, attendance at additional complex case conferences, and more intensive brokerage support.
